Home Health Registered Nurse – Weekend Visits

Hartford Healthcare seeks a compassionate Weekend Revisit Nurse to deliver high-quality follow-up care across our network. In this field-based role, you will perform weekend revisit assessments, manage medications and wounds, and update care plans to support safe discharges and reduce readmissions. Perform weekend revisit nursing assessments and interventions in patient homes or care facilities Update care plans, administer medications, and provide wound and symptom management Document care accurately in the EHR and communicate changes to the interdisciplinary team Educate patients and families on disease management, medications, and follow-up care Collaborate with providers and care coordinators to support safe discharges and prevent readmissions Adhere to infection control, safety, and quality standards across all visits Patient assessment Care planning IV therapy and medication administration Wound care management Electronic health record (EHR) documentation Home health visit management Care coordination and communication Patient and family education Infection control practices Clinical triage and prioritization Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice BSN preferred BLS certification ACLS preferred Experience in home health or community nursing preferred
