Work Environment: This position is onsite and centered around home care, providing you with opportunities to work closely with patients in the comfort of their own homes. Your typical working hours will be Monday to Friday from 8:00 AM to 5:00 PM. This environment allows for a personalized approach to therapy, fostering meaningful connections that facilitate rehabilitation and recovery.

Key Responsibilities: Adhere to state practice acts and laws governing physical therapy, ensuring compliance with all regulations while delivering patient care. Conduct comprehensive evaluations of patients with functional limitations and disabilities, developing personalized treatment plans grounded in clinical assessment. Utilize evidence-based practices to implement treatment protocols, monitoring responses and adjusting plans as needed based on patient progress. Document treatment plans, patient progress, and relevant findings accurately and promptly, creating a clear and legible record of care. Collaborate with other healthcare professionals to ensure comprehensive care and address evolving needs, modifying plans of care in response to patient changes. Engage in appropriate discharge planning and follow-up care to optimize patient outcomes. Participate actively in facility meetings and team initiatives aimed at removing barriers to access and improving quality metrics. Patient Population / Client FocusIn this role, you will work mainly with patients requiring home care therapy due to a range of orthopedic conditions. Your clients will include individuals recovering from surgeries, managing chronic pain, and experiencing mobility challenges. You will utilize a compassionate approach to nurture a supportive environment that encourages healing and recovery.

Qualifications: Doctorate of Physical Therapy (DPT) from an accredited physical therapy program. Current licensure as a Physical Therapist in the state of New Jersey. A minimum of two years of experience in outpatient orthopedic therapy, showcasing a commitment to patient-centered care. Current CPR certification to ensure patient safety in all care scenarios.

Preferred Experience: Experience working in home care settings, with a demonstrated ability to provide effective treatment in the patient's living environment. Familiarity with electronic health record (EHR) systems for documenting patient care and managing scheduling efficiently.
